The switching of endpoints, the fact that ivermectin was also given to placebo (this data was removed when found), that the study was done in Cali Colombia where ivermectin use was endemic (which may explain why the placebo arm seemed to do better than they planned for) - study participants were also young and unlikely to show severity (further diluting the disparity between the arms).
